Chemistry at Work™ Participant Terms

Chemistry at Work™ is a workforce health education program provided by Good Health Geek (“GHG”). By enrolling in or participating in the program, you acknowledge and agree to the following terms.

1. Educational Purpose

Chemistry at Work™ is provided solely for educational and informational purposes. Program materials are intended to help participants learn about nutrition, nutrients, lifestyle factors, biological processes, and factors that may influence everyday health, wellbeing, energy, focus, mood, and performance.

Information provided by GHG is not medical advice and is not intended to diagnose, treat, cure, prevent, or manage any disease, disorder, or medical condition.

Participation in Chemistry at Work™ does not create a physician-patient, dietitian-patient, therapist-patient, or other healthcare provider relationship between you and Good Health Geek.

GHG does not practice medicine, diagnose medical conditions, prescribe medications, order medical treatment, or provide individualized medical treatment through Chemistry at Work™.

2. Medical Decisions and Professional Care

Program information should not be used as a substitute for care or advice from a qualified healthcare professional.

You are responsible for decisions concerning your health and should discuss symptoms, medical conditions, medications, supplements, dietary changes, abnormal test results, or other health concerns with an appropriately qualified healthcare professional.

Do not delay or discontinue medical care because of information presented through Chemistry at Work™.

If you believe you are experiencing a medical emergency, seek emergency medical assistance immediately.

3. Testing Services Are Provided Independently

Certain Chemistry at Work™ program options may provide participants with access to laboratory, screening, specimen-collection, phlebotomy, or related testing services.

Good Health Geek does not perform laboratory testing, collect or analyze specimens, operate a laboratory, provide phlebotomy services, establish laboratory procedures, determine laboratory results, or control the professional services of independent testing providers.

These services are performed by separate, independent companies and professionals that are solely responsible for their own services, personnel, procedures, testing methods, accuracy, quality controls, reporting, regulatory compliance, privacy practices, and professional obligations.

Any questions, complaints, concerns, disputes, adverse experiences, billing matters, specimen issues, test-result questions, or service-related concerns involving a testing, laboratory, or specimen-collection provider should be directed to that provider.

GHG's inclusion of or arrangement for access to an independent provider does not constitute a guarantee of that provider's services or results.

4. Dietitian and Nutrition Professional Services Are Independent

When access to a registered or licensed dietitian or other nutrition professional is included or made available through Chemistry at Work™, those professional services are provided independently from Good Health Geek.

Dietitians and other licensed professionals exercise their own professional judgment and are solely responsible for the advice, recommendations, assessments, services, and professional care they provide.

GHG does not direct, supervise, control, or assume responsibility for an independent professional's clinical judgment or recommendations.

Any questions, complaints, concerns, professional-care issues, scheduling matters, billing matters, or disputes concerning dietitian or other professional services should be directed to the company or professional providing those services.

5. Third-Party Providers

GHG may coordinate access to independent laboratories, testing companies, specimen-collection services, dietitians, technology platforms, or other third-party service providers.

These organizations are separate legal entities and are not employees, agents, healthcare providers, subsidiaries, or representatives of GHG unless expressly stated otherwise in writing.

You may be required to accept separate consent forms, privacy notices, terms of service, or other agreements directly with those providers. Your relationship with each provider is also governed by that provider's applicable terms and policies.

6. Supplements, Nutrition and Lifestyle Information

Any discussion of foods, nutrients, dietary supplements, exercise, sleep, stress, or other lifestyle practices within GHG educational materials is general educational information unless provided independently by a qualified professional acting within that professional's scope of practice.

GHG does not recommend that participants begin, discontinue, increase, decrease, or replace prescribed medications or medical treatment.

Individual nutritional and supplement needs vary. Supplements can interact with medications, medical conditions, laboratory testing, and one another. Participants should consult an appropriately qualified healthcare professional before making significant changes when individual medical circumstances warrant professional guidance.

7. Participant Responsibility

You acknowledge that health and biological responses vary between individuals and that no particular health, wellness, performance, or other outcome is promised or guaranteed through participation in Chemistry at Work™.

You remain responsible for your own healthcare decisions and for providing accurate information to any independent healthcare professional or service provider you choose to use.

8. Privacy and Employer Separation

Good Health Geek does not receive, access, review, maintain, or store individual participant laboratory results, testing data, or other individual health information generated through independent testing providers.

Individual test results and related health information are provided directly by the independent testing provider to the participant. The testing provider is independently responsible for the collection, use, storage, protection, and disclosure of information in accordance with its own privacy policies and applicable law.

Any information provided directly to a dietitian or other independent healthcare professional is handled by that provider in accordance with its own privacy practices and applicable professional and legal requirements.

Participants should review the privacy notices, consent forms, and authorizations provided by each independent provider before using its services.

9. Concerns Regarding Third-Party Services

GHG welcomes feedback regarding the overall Chemistry at Work™ program. However, GHG does not investigate, adjudicate, or assume responsibility for professional services delivered by independent providers.

Concerns involving laboratory procedures, specimen collection, testing accuracy, test results, dietitian services, professional recommendations, privacy practices of a third-party provider, or other independently provided services should be addressed directly with the organization responsible for that service.

10. Limitation of Liability

To the fullest extent permitted by applicable law, Good Health Geek and its owners, officers, directors, employees, contractors, representatives, and affiliates shall not be liable for claims, losses, damages, injuries, costs, or expenses arising from or relating to services, acts, omissions, professional judgment, testing, specimen collection, laboratory analysis, results, recommendations, products, technology, or other activities provided by independent third parties.

To the fullest extent permitted by applicable law, GHG shall not be responsible for decisions or actions taken by a participant based upon educational information provided through Chemistry at Work™, including decisions concerning diet, supplementation, lifestyle practices, testing, or healthcare.

Nothing in these Terms excludes or limits liability that cannot lawfully be excluded or limited under applicable law.

11. No Guarantee of Results

GHG makes no guarantee that participation in Chemistry at Work™, testing, educational activities, dietitian services, or implementation of information learned through the program will produce any particular health, wellbeing, performance, employment, or other outcome.

Individual results and experiences will vary.

12. Intellectual Property

Chemistry at Work™, its educational materials, videos, graphics, assessments, interactive experiences, guides, methodologies, and related content are owned by or licensed to Good Health Geek and are protected by applicable intellectual-property laws.

Program materials are provided for the participant's personal educational use and may not be reproduced, distributed, sold, published, recorded, or used commercially without written authorization from GHG.

13. Voluntary Participation

Unless otherwise expressly communicated by your employer, participation in Chemistry at Work™ is voluntary.

You may choose whether to participate in optional testing, dietitian services, or other independent services made available in connection with the program.

14. Electronic Agreement

By checking the acknowledgment box presented during registration and submitting the form, you confirm that you have read, understand, and agree to these Participant Terms, Educational Disclaimer & Acknowledgment.

Your electronic acceptance constitutes your agreement to these Terms and is considered executed on the date and time your acceptance is electronically recorded.

15. Governing Terms

These Terms shall be interpreted and enforced in accordance with applicable law. If any provision is determined to be invalid or unenforceable, the remaining provisions shall remain in effect to the fullest extent permitted by law.

These Terms may be supplemented by additional program-specific terms, privacy notices, or agreements presented to you during participation.
